Skills and Requirements:
Preferably have one year of experience in Discharge Planning.
A recent work experience in adult cancer unit of minimum three years.
Committed to caring and showing compassion for cancer patients.
Knowledge of medical/nursing standards of care.
Ability to create and manage a patient discharge database and analyze data collected.
Ability to effectively collaborate with nurses, physicians, patients/families and ancillary staff.

Roles and Responsibilities:
Responsible for coordinating the necessary care services to meet the patient’s needs to ensure a safe discharge.
Function as the point of contact for all information related to the patient’s discharge.
Manage all post hospital care and follow-up through coordination of patient’s discharge planning with multi-disciplinary teams including attending inter-disciplinary team meetings, huddles and initiating consults in support of patient’s discharge.
Assist in securing transportation upon discharge as needed.
Demonstrate experience in identifying and providing patient centered care of high-risk patients.
Make daily rounds on units assigned to meet newly admitted patients.
Coordinate and conduct discharge planning meetings on all inpatient units.
Create and manages a patient discharge database and analyze data collected.
Function as a liaison between patients and/or other health care facilities, other members of the disciplinary team and community health services to ensure continuity of care.

- Pain Control/Management Nurse
Qualifications (A minimum of):

Specialized certificate or Masters in Pain Management (adult).

Roles and Responsibilities:
Work in different cancer clinical programs providing nursing pain management care to cancer patients.
Provide high-quality health care through direct assessment, diagnosis, and formulation/implementation of treatment plan for patients with common acute and chronic Pain.
Write initial nursing histories, assess patient’s conditions and assist with the development of individual care plans for patients.
Assist physicians with examinations, procedures and other processes related to direct patient care.
Provide health care instruction/education to patient, family members or designated caregiver.
Communicate effectively with physicians, nurses and other personnel in problem identification and resolution in a timely manner.
Work in conjunction with the leadership staff and staff/physicians, in identifying areas of opportunity for improved efficiency and improvement in quality care delivery utilizing the Performance Improvement process.
Plan, organize, implement and evaluate the delivery of care in accordance with standards of care.
Assist in the development and implementation of ongoing educational programs for professional and support staff.
Direct and monitor the instruction, supervision and evaluation of students and new employees.

Qualifications:
BSc in Nursing + MSc in Nursing (Palliative Care or Cancer)
Bachelor of Nursing + Diploma / Professional Certificate of not less than (9) nine months or equivalent in Palliative Care or Cancer Care
Skills and Requirements:
Recent work experience of at least 5 years in an adult cancer center.
Specialized certificate or Master in Palliative Care (adult) or Cancer/Oncology.
Committed to caring and showing compassion for cancer patients.
Knowledge of medical/nursing standards of care.
Ability to effectively collaborate with nurses, physicians, patients/families and ancillary staff.

Roles and Responsibilities:
Complete initial and on-going comprehensive physical and psychosocial assessment of the patient and family to identify needs and areas for intervention.

Implement and evaluate patient care plans based on patient assessment to optimize outcomes and maximize available resources.

- Stoma/Wound Care Nurse
Qualifications:
BSc in Nursing + MSc in Nursing (tissue viability/ wound care)
BSc in Nursing + Diploma/specialization certificate not less than 9 months or equivalent+ 4 years of experience
Skills and Requirements:
Recent work experience of at least 3 years in a cancer center or a tertiary care level hospitalor higher.
Specialized certificate or Masters in Wound Care or Stoma Care.
Excellent (verbal and written) communication skills.
Computer skills, including email, internet and Microsoft Office programs.
Ability to effectively collaborate with nurses, physicians, patients/families and ancillary staff.
Ability to act professionally under pressure.

Roles and Responsibilities:
Provide care to patients requiring wound or stoma care.
Meet with elective patients pre-operatively to give information and education regarding proposed surgery and stoma formation.
Provide guidance, support, advice and counseling to patients and their family/caregivers.
Promote patient autonomy where possible through education and support so that the individual is able to make informed choices about their own care.
Recognize when patients are unable to manage their stoma care independently, either physically or psychologically and actively formulate an alternative plan of care.
Act as the patient’s advocate when required.
